Our mission

Improved communication and collaboration in hospital settings

Care Connector was developed in 2015 at Trillium Health Partners’ Institute for Better Health (IBH).  The goal was to allow physicians, nurses, social workers, and other clinical staff working in teams to provide more effective and efficient care for the increasingly complex patient population in acute care settings. Over 2,400 frontline clinicians guided the design to ensure the solution addressed gaps in existing communication practices.

In 2023, Careteam Technologies, a cutting-edge physician-led Canadian digital health company with a focus on integrated care, acquired Care Connector. The combination of both the Careteam and Care Connector platforms will enable a more streamlined and integrated experience for patients as they move from an acute care setting to home or community care - along the continuum of care.

Careteam, created by Careteam Technologies, is an innovative collaboration platform that brings the patient, personal support team and health care providers together with an integrated care plan, instant communications, and population health analytics to better enable care transitions and complex chronic care management in an often fragmented health care environment outside of acute care settings.
